bluenhs.org corrects verified factual errors promptly and says so. To request a correction or submit a right of reply, write to rafal@ctozen.com. Every request is acknowledged within five working days. Where a factual error is confirmed, the text is corrected and a dated note is added to the log on this page. Named individuals and organisations may submit a reply, which will be considered for publication alongside the passage it concerns.
Our approach
The Digital Blueprint for NHS Wales is built from the public record: board papers, Senedd committee reports, audit findings, Welsh Government publications and published journalism. Each factual claim is meant to trace back to a document anyone can read. The full list is on The 89 Sources page, and individual pages carry their own source notes.
The analysis draws conclusions from that record, and those conclusions are open to challenge. The facts underneath them must be right. If they are not, the site will correct them.
Requesting a correction
If you believe something on this site is factually wrong, write to rafal@ctozen.com. It helps to include:
- the page address (URL) and the passage concerned
- what you believe is incorrect
- the correct information, with a source or document that shows it, where one exists
Right of reply
If you are a named individual, or you represent an organisation named on this site, you may submit a reply to any passage that concerns you. Send it to the same address, saying which page and passage it relates to.
Replies will be considered for publication alongside the relevant passage, attributed to you or your organisation.
What happens next
- Acknowledgement within five working days of receiving your message.
- Review against the record. The claim is checked against the cited source and any evidence you supply.
- Prompt correction of verified factual errors. The text is changed on the site and a dated note is added to the log below.
- An explanation if no change is made. If the original text is supported by the record, you will be told which source supports it.
Differences of interpretation are not treated as factual errors, but they are a fair subject for a right of reply.
Corrections log
Corrections are listed newest first. Typographical fixes, broken-link repairs and clarifications that do not change a factual claim are not logged.
6 June 2026. A phrase presented as the wording of the Senedd Public Accounts Committee’s November 2018 report on NWIS (“antithesis of open”) was a paraphrase, not the Committee’s words. It was replaced across the site with the verbatim wording from the Chair’s foreword: “a culture of self-censorship and denial” and “a pre-prepared line”. Pages affected: Why This Blueprint, Applying This To DHCW and L4: The Rebranding Escape.
4 June 2026. Two claims were withdrawn across the site. First, the statement that DHCW was the first NHS Wales body to be placed at Level 3 enhanced monitoring: other NHS Wales bodies have been escalated higher and for longer, and the claim of primacy could not be supported. DHCW’s own escalation record is unchanged. Second, the statement that “three” executive directors received University of Wales Trinity Saint David Professor of Practice titles in December 2020: the count was removed. The documented record of named declarations of interest is unchanged.
